Microix Form Designer| Custom Report Designer
- Click on "Form Designer" in the Maintenance tab.
- A new tab displaying the existing forms in the system will open on the user's screen.
- Select the "Requisition" option from the drop-down list for the Module field. All the purchase requests in the system will be listed on the screen. (See screenshot below.)
If the client needs Microix to assist with customization/modifications to a form, please submit a copy of the form for approval first. A minimum fee of $1,500 for a custom form change can be added to the engagement once approved by Microix.

Q: With the form designer, are we limited to forms related to the functions within Microix? For example, would we be able to create a form for an employee training, where the employee would be able to request to go to a training or conference and be approved thru a workflow? Something else unrelated to this list?
A: It is only within the functions of the software, as the form designer links with the interfaces for each function. Clients can still submit a request for training through the PO module, however, unable to have a form that has more information than what is present in the PO interface.
