Requisition Module | Sample Requisition/PO Form

We recommend using the default PO form which includes most of the information related to a standard requisition/PO form. However, if you need modifications, it can be considered but we cannot guarantee it will look the same as your old form. We can add to our existing forms if the data is available in Microix.  If it is not available in Microix, a text box can be added to the form for customer to manually enter after the form Is printed. A minimum fee of $1,500 for custom form change can be added to the engagement.
